BI-RADS is an acronym for Imaging and Breast Imaging Systems, a quality assurance tool originally designed for use with mammography. This system is a collaborative effort of many health groups but is published and trademarked by the American College of Radiology (ACR).
The system is designed to standardize reporting, and is used by medical professionals to communicate the risk of breast cancer patients. This document focuses on patient reports used by medical professionals, not "lay reports" given to patients.

Published Documents
BI-RADS is published by ACR in the form of BI-RADS Atlas. In 2013 Atlas is divided into 3 publications:
- Mamography, Fifth Edition
- Ultrasound, Second Edition
- MRI, Second Edition

Rating Categories
While BI-RADS is a quality control system, in everyday use the term "BI-RADS" refers to the category of mammography assessment. This is the standard numerical code usually assigned by a radiologist after interpreting a mammogram. This allows a concise and unambiguous understanding of patient records between many doctors and medical facilities.
Assessment categories were developed for mammography and then adapted for use with MRI and ultrasound findings. The summary of each category, given below, is almost identical for all 3 modalities.
Category 6 is added in the 4th edition of BI-RADS.
Assessment Category BI-RADS are:
- 0: Incomplete
- 1: Negatives
- 2: Run
- 3: Probably tame
- 4: Suspicious
- 5: Highly suggestive about violence
- 6: Known biopsy - proven terror
The incomplete classification (BI-RADS 0) guarantees an attempt to ensure prior imagery for comparison or to call the patient back for additional views and/or higher quality films. The BI-RADS classification of 4 or 5 guarantees a biopsy to further evaluate the offending lesion. Some experts believe that the BI-RADS 4 classification does not adequately communicate cancer risk to doctors and recommends a subclassified scheme:
- 4A: low suspicion for malignancy, about 2%
- 4B: suspicion of medium-sized malignancy, about 10%
- 4C: moderate concern, but not classic for malignancy, about 50%
Breast Composition Category
Start the 5th edition of BI-RADS
- a. Breasts are almost entirely fatty
- b. There is a dispersed area of fibroglandular density
- c. Her breasts are heterogeneously solid, which may obscure a small mass
- d. Her breasts are very dense, which decreases the sensitivity of mammography
Auto Extraction
Automatic parsers have been developed to automatically extract BI-RADS features, categories, and compositions from textual mammography reports.
