Primary peritoneal cancer or carcinoma is also known as surface serousis papillary carcinoma, primary peritoneal carcinoma, extra-ovarian serous carcinoma, serous primary papary carcinoma, psammomacarcinoma. It has historically been classified under "primary carcinoma unknown" (CUP). Primary peritoneal cancer (PPC, or PPCa) is a cancer of cells that line the peritoneum, or abdominal cavity.

Several studies have shown that up to 15% of ovarian serous cancers are considered primary primary peritoneal carcinomas.

Histomorphological and molecular biological characteristics indicate that serous carcinoma, which includes ovarian serous carcinoma, serous carcinoma of the uterus, Fallopian tube serosal carcinoma, serous carcinoma of the cervix, and primary serous peritoneal carcinoma actually represent one entity.

Theory

The epithelium of the ovary and the peritoneum share the same embryonic origin, the coelomic epithelium (mesodermal origin). Coelomic epithelium is thought to be derived from mesonephrine. With the overall point is that the normal ovarian and peritoneal tissue comes from mesonephros. In contrast, the fallopian tube epithelium, endometrium and endocervix are associated with paramesonefros (Müller's duct). Curiously, epithelial ovarian cancer and primary peritoneal cancer are histologically similar to the Mullerian epithelium; not their embryonic origin, mesonephros. Either the metaplasia has occurred or the remains of Mullerian have been left in the coelemic epithelium, which has become oncogenic.

Genetic Causes

Although the exact cause is unknown, links with certain variants of BRCA1/2 have been described. Furthermore, women with BRCA1/2 mutations had a 5% risk to develop primary peritoneal cancer even after prophylactic oophorectomy.

Primary peritoneal carcinomas show similar rates of tumor suppressor gene dysfunction (p53, BRCA, WT1) as ovarian cancer and may also show an increase in HER-2/neu expression.

The association with vascular endothelial growth factor has been observed.

Prognosis and treatment

Prognosis and treatment are similar to the most common type of ovarian cancer, epithelial ovarian cancer.

The average survival of primary peritoneal carcinoma is usually shorter with 2-6 months when compared with serous carcinoma. Studies show average survival varies between 11.3-17.8 months. One study reported an average survival of 19-40 months (95% CI) with a 5.5 year survival of 26.5%.

Increased levels of albumin are associated with a better prognosis.
